What is the purpose of this project?

The purpose of this project is to deliver high quality, efficient, integrated services through a partnership approach.  The project will test new ways of providing services with and for older people. With the right support closer to home, older people can avoid hospital or care home admissions, so they will be supported to be healthy and active, independent, have regard for their well-being and to participate fully in the community.

Who is involved?

This organisations involved in this project and working together in partnership are Adult and Community Services at Devon County Council, Devon Primary Care Trust, Devon Partnership NHS Trust, South Devon Healthcare NHS Foundation Trust, Northern Devon Healthcare NHS Trust, South Western Ambulance Services NHS Trust, the Royal Devon and Exeter NHS Foundation Trust and the Voluntary Sector.

What happens when the funding runs out?

The funding for this project ends in April 2009.  The integrated health and social care teams will have their own joint budgets.  We hope that the success of the Community Mentoring service will mean that the service will be commissioned and offered through GP surgeries.  The Foot Care service will be a chargeable service and so will be self sustaining.

How will the project help?

The project will help in four main ways;

    • Arranging Community Mentoring services to help vulnerable and socially isolated people to get back into mainstream life.
    • Creating Multi-Disciplinary Teams – Health and Social Care Teams, working together in clusters based in market and coastal towns near to where people live.
    • Funding for development work which will provide ongoing support for Carers.
